The technical gains land first, usually within weeks of the rebuild going live: speed, crawlability, structured data and the map results. Ranking gains build over the months after that. Anyone promising you page one by a specific date is guessing.
Not always, but usually. If your site is fast, properly marked up, has no broken URLs and search engines can already read it, then the work is content and we will tell you that. Most sites we look at fail at least two of those, and no amount of writing fixes it.
No. Every existing URL is mapped and redirected as part of the build, so nothing 404s and nothing loses the history it has built up. Losing rankings in a redesign happens when nobody does that mapping, which is exactly the step most rebuilds skip.
